You can mail it to GCHNA, PO Box 1343, Nevada City, CA 95959, or deliver it to Joy Elson. It must be printed and filled out by hand (a digital signature is not accepted by OES).

About Evacuation Routes
If you live along Cement Hill Road, there is only one Emergency Fire Exit Road, which connects the end of Cement Hill Road with Cedarsong Road, then down to Highway 49. This road is on private property and ONLY to be used in the case of an emergency evacuation situation.
A wildfire is unpredictable – evacuating early is one thing you can control.
